An inverted hierarchy ranks elimination, substitution, engineering controls, administrative controls and personal protective equipment from generally most to least effective. Elimination removes the hazard at source. Substitution replaces it and requires review of new risks. A separate assessment and review loop supports selecting, combining and checking controls; it does not require choosing only one tier.
